Declaration: I hereby apply for membership of the New Plymouth Taekwondo & Hapkido Club (known as NPTHC). I further more agree to abide by the following conditions:
1. I will abide by the Club Rules and the rules of any organisation the club is affiliated too.
2. The instructor has the right to withhold tuition from me if I disturb the class or breach any of the rules.
3. I hold myself responsible for any injuries I may sustain during my training.
4. I agree to not misuse the knowledge gained through my instruction.
5. I agree to wear the personal protection equipment (PPE), and or mouth guard when instructed to do so.
6. From time-to-time photos and videos will be taken for promotional purposes, I will let the instructor know if I do not want to be caught on film/photo.
